2900 W. 93rd Avenue, Crown Point, IN. 46307 (located across from government complex). Monday-Friday, 9a.m.- 4p.m. Appointments are not needed.
They serve all patients regardless of insurance status: private, state, or uninsured/underinsured. Call ahead for insurance eligibility for private insurance.
Please bring an Indiana Medicaid or current, insurance card, if your child has one. Please bring a current shot record for your child, especially if you are from out of state.
